Surplus Pickup and Disposition Request Form
Contract FAC42
Please use this form to request a pickup of surplus
property under Mass. Operational Services Division Contact No. FAC42.
This contract is open to Massachusetts state agencies, municipalities
and school districts, and non-profit organizations.
The Institution Recycling Network is committed to assuring
that all surplus managed under this contract will be donated to U.S.
and international
relief agencies, who will use Massachusetts state surplus for disaster
relief and economic development. Most surplus disposed under the
contract will be used for hurricane relief for Hurricanes Katrina and
Rita and
in areas of the Caribbean devastated by 2004 and 2005 hurricanes.
If surplus cannot be donated for relief efforts, it will be recycled
for
its wood or metal content. No surplus disposed through contract
FAC42 will be landfilled or incinerated.
When you submit this form, IRN will contact you within
three business days to discuss details and prepare a proposal for
service. Users
of contract FAC42 are asked to cover the costs for pickup and in-state
transportation of surplus, consolidation, and preparation of outbound
shipments. IRN’s nonprofit partners absorb all costs after this
point, including land and sea transportation, warehousing, identifying
and coordinating with local receiving organizations, and distribution
to recipients. Overall, the cost to manage surplus through this
program is almost always less than the cost of simply throwing it
away.
Filling out the form, the most important thing is to
provide as accurate an inventory as you can of the items you have
as surplus.
To keep costs
as low as possible, we will set up “milk run” collection
routes to pick up surplus from several generators at a time, and
to plan effectively, we need a good estimate of the type and quantity
of
material you will be providing.
